If it is a small bell up front, it is one of the cash register lines. In this case, the person answering goes up to the front, calls out 'two bells', and the person making the request raises his hand.
If the bells sound incoherent, we know that someone's child is just playing with the bell.
Siri types my posts for me.
In the TJ's I go to if the cashier rings the bell sometimes help will arrive and say "Who's ringin' that bell!?" in a friendly jovial kind of way. I don't know why, but it does make me smile and usually I'm a no fuss in and out shopper.
